Required Enrollment Forms
The Baltimore American Indian Center accepts children into our program without regard to
race, creed, color, national origin, sex or disability.
Simply print and fill out these forms, and return them to
our facility one week prior to your child/ren entering the program. If you
have any questions regarding any of these forms, which are required by the
State of Maryland, please feel free to contact the Director.

Form 896:
Maryland DHMH Immunization Certificate
Please read the directions on page 2 of this form carefully, complete and
have it signed by your doctor.
Form 1214: Emergency Form
Complete contact information for parents and an alternate such as a
grandparent or family friend. Also outlines any medical conditions.
Form 1215 and
Form 1215a: Health Inventory
A complete assessment of your child's health, including screening for lead.
Requires the signature of a health professional.

Detailed instructions for filling out form 8506. Worth the time to read.

return a form stating that they have received it.

This form is required of all Vera Shank parents, as all students can be seen via the Video Gateway.
Optional and Occasional Forms
Form 1216: Medication Order Form
Must be completed and returned if your child is to receive medication at our
facility, whether the order is ongoing or temporary.
Permission Slip
A standard Permission Slip for field trips and special activities.
